Filing a Complaint


It is the policy of the City of Lebanon to encourage citizens to bring forward concerns regarding misconduct by City personnel and officials. This page outlines the procedures for receiving, investigating, and processing complaints regarding City personnel and officials. The process for the filing of complaints and the investigation of those complaints is outlined in Policy ADM-126.1: Complaints & Investigations

Submitting Complaints

Complaints can be submitted online using the City's Online Complaint Form. Please provide as much detailed information as possible to aid in the investigation of your concerns. 

Submitting Anonymous Complaints

To submit an anonymous complaint, please use our Anonymous Complaint Form hosted by Syntrio Lighthouse Services or call 877-990-0006. The City will act on anonymous complaints according to its Anonymous Hotline Policy, however, we may be limited in our ability to investigate those concerns depending upon the information provided. 

Complaints against the City Manager

Please be aware that complaints against the City Manager will be routed to the City of Lebanon Mayor.
